Description:
This class is required for Service Coordinators, Foster Providers, Residential & Vocational Staff who may need to assist individuals with intellectual and developmental disabilities to obtain an ISP-Team Appointed Health Care Representative. ODDS requires that one ISP team member PLUS the individual’s DD Service Coordinator must have completed this ODDS HCR Training Course before an ISP HCR appointment can be made.
Why would someone need an HCR? If people with intellectual/developmental disabilities are not capable of making their own healthcare decisions, they can choose someone to make decisions for them. If they are unable to choose someone, the ISP team can choose a qualified person as the HCR for times when surgery is being considered, a person’s health is failing, or they need multiple health supports. This class also covers POLST, Advance Directives, HIV Testing, and feeding tubes.
